The Board of Directors of the Ordre des dentistes du Québec has established a Compensation Fund intended to compensate individuals following the use, by a dentist, of a sum for purposes other than those for which it was entrusted to them.
How to file a claim
If you wish to file a claim, you must complete the form available online and sign it in front of a person authorized to administer oaths (lawyer, notary, or commissioner for oaths).
If the space is insufficient, you may attach additional pages.
To ensure your claim is processed, the form must be fully completed, signed, and accompanied by all supporting documents or evidence (copies of treatment plans, invoices, cheques, receipts, statements of account).
If your claim is incomplete, it will be returned to you along with an letter.
Note : Damages and interest cannot be claimed from the Compensation Fund of the Ordre des dentistes du Québec. If applicable, such claims may be pursued through civil court proceedings.
Deadline to file a claim
The claimant has 12 months from the date he became aware that the amount has been used by a dentist for purposes other than those for which it was entrusted to them.
