If you are dissatisfied with the quality of professional services provided by your dentist, we suggest that you contact them to discuss this in an open and respectful manner. Giving your dentist the opportunity to provide explanations and answer your questions often resolves the situation and dispute.

If in doubt or to obtain additional information before requesting an inquiry, please consult the page Frequently asked questions, where you will find useful and relevant information. If you would like clarifications and wish to discuss a situation with us, please contact the Office of the Syndic at:

  • 514 875-8511, ext. 2225 (toll-free : 1 800 361-4887).

The inquiry request must be in writing and submitted to the attention of the Syndic. It must contain sufficient detail, in particular, as to the nature of the allegations, to give reason to believe that a breach of ethical obligations has occurred, and thus justify an inquiry of a dentist.

ATTENTION : IF YOUR INTENTION IS TO OBTAIN FINANCIAL COMPENSATION

Please note that sending an inquiry request to the Office of the Syndic, holding an inquiry, and if applicable, the disciplinary process before the Disciplinary Council of our professional order do not enable you to obtain financial compensation for dental malpractice.

If you wish to obtain financial compensation, you must contact the dentist directly before waiting for the findings of the inquiry. If no satisfactory agreement is reached, you must turn to the civil courts to exercise your rights.

We encourage you to consult a lawyer to advise you and help you exercise your rights. You can also consult the website (in French only) : Fonds d’assurance-responsabilité professionnelle de l’Ordre des dentistes du Québec.

If you would like to file a complaint with the Office of the Syndic, you have three options.

  • Name and contact information of the dentist in question
  • Description of the events giving rise to the complaint, including dates and locations
  • The dentist’s alleged conduct
  • If applicable, the name and contact information of other dentists or professionals consulted
  • Name and contact information of any other person involved or any witness of the events
  • Any documents you have that support your allegations
  • Your name and contact information, including an email address and phone number where you can be reached during the day

You can send your inquiry request to [email protected] or mail it to :

Bureau du syndic
800 René-Lévesque Blvd. West
Suite 1640
Montreal, Quebec  H3B 1X9

Exceptionally, a report can be made anonymously, verbally or in writing, provided that the information provided is detailed enough to enable a Syndic to undertake an inquiry; notably the alleged conduct and the name of the dentist and their contact information, as well as the name and contact information of the patient involved and, if applicable, any witnesses to the events.
